Tranexamic acid in acute traumatic brain injury

In patients with moderate traumatic brain injury (TBI) (Glasgow Coma Scale greater than 8 and less than 13) presenting within three hours of injury, immediately administer tranexamic acid (Grade 1B).

In the CRASH-3 trial, among 9000 patients presenting within three hours of TBI with a Glasgow Coma Scale (GCS) < 13 or any evidence of intracranial bleeding on CT scan, those receiving tranexamic acid had a risk of head injury-related death that was non-significantly lower relative to placebo. This difference was statistically significant when patients with unreactive pupils were excluded and in the subgroup of patients with mild to moderate TBI. ....read more

DoP to create Pharma Bureau to facilitate ease of doing business in India through single window clearance

In order to facilitate ease of doing business in the Indian pharma industry, the Department of Pharmaceuticals (DoP) is in the process of creating a Pharma Bureau to allay concerns of the investors both local and foreign who have to approach multiple agencies under the Central Government for setting up manufacturing facilities in India. This is very much required as the industry has been struggling to get coordinated support from agencies like the Drug Controller General of India (DCGI), Directorate General of Health Services (DGHS), Departments of Health, Pharmaceuticals, Environment, Commerce, Finance, Revenue, Bio-technology, Department for Promotion of Industry and Internal Trade (DPIIT), Labour, Ministry of Micro, Small and Medium Enterprises (MSME), Chemicals and Petrochemicals. Pharma Bureau will help remove the bottlenecks and provide policy support and mechanism to the industry for its sustainable growth. “Setting up of a Pharma Bureau will address concerns and problems of the pharmaceutical industry under one roof. This will save valuable money, time and ensure uninterrupted production of medicines and medical devices for introduction of newer and effective therapies for the patients,” informed Secretary, DoP, P D Vaghela.

Industry urges govt to defer implementation of newly inserted Rule 36(4) in CGST till April 1, 2020

The pharmaceutical industry in the country has urged the union government to defer the applicability of newly inserted Rule 36(4) in Central Goods & Services Tax (CGST) till introduction of new system of invoicing which is scheduled to be applicable from April 1, 2020. The industry cited non-operational system of matching as a reason for deferring the implementation of Rule 36(4). Government must act pro-actively and its applicability be deferred along with new system of invoicing, it said. The government has recently made rule to restrict input tax credit (ITC) of invoice, debit notes not reflecting in GSTR-2A of receiver to 20 per cent of eligible ITC reflecting in GSTR-2A. The newly inserted Rule 36(4) provides that a taxpayer can avail ITC pertaining to outward supplies not declared by his supplier in Form GSTR-1 only to the extent of 20% of the eligible credit available in respect of invoices declared by his supplier in Form GSTR-1 which is reflected in Form GSTR-2A. This rule was inserted with effect from October 9, 2019. This newly inserted provision along with circular issued by Central Board of Indirect Taxes and Customs (CBIC) has created lot of hue and cry amongst taxpayer. Calculation of Rule 42/43 reversal will suffer a lot, since now all calculation will be based on ITC as per Rule 36(4) rather than actual ITC. Taxpayer may require working capital at earlier and accumulation of ITC at later stage, said Indian Drug Manufacturers’ Association (IDMA).

The first heart transplant at Sawai Man Singh (SMS) Hospital on Thursday has come as a ray of hope for 21 patients who have applied for it and awaiting for their turn to get a new lease of life. The cardiologists have listed these patients as candidates requiring heart transplant that their heart is functioning below 20% of the capacity. On Thursday, hospital doctors had retrieved a heart from a 25-year-old braid dead person and transplanted it into a 17-year-old patient suffering from heart failure. “The recovery of the heart recipient is satisfactory,” said Dr Anil Sharma, head of the department (cardiothoracic surgery). Jaipur has become a prominent centre in the country for heart transplants as three hospitals — two private and a government — have already performed five heart transplants in the past four years. According to the State Organ & Tissue Transplant Organisation (SOTTO), the waiting list of patients requiring organs transplant in web registry of Rajasthan Network for Organ Sharing (RNOS) are as follows: 320 patients for kidney transplant, 43 liver transplant and 21 heart transplant. With increasing need of patients suffering from chronic kidney, liver and heart diseases, SMS Medical College has started preparations for expanding its facilities for organ transplant. While giving organ donation a major impetus, the hospital is constructing a multi-storey building near its Institute of Traumatology and Orthopedics (ITO) wing and a Centre for Excellence for Organ Transplant at one of the floors of the hospital building.
