Derma Update: Differences are present between childhood-onset and late-onset atopic dermatitis disease

Globally, atopic eczema is one of the most predominant diseases and a common dermatological disorder. Existing data shows that the occurrence of the disease is still increasing, in children and adults, especially in low-income countries. Atopic dermatitis in adults involves childhood-onset (COAD) and adult or late-onset types (LOAD). Earlier research has defined the differences in clinical features, laboratory data, and response to prescribed treatment between COAD and LOAD. Many studies are done on an ambulatory population, which generally includes patients with mild to moderate disease.

The objective of this study was to define the differences or alterations between adults hospitalized with COAD and LOAD. Data were evaluated from a retrospective cohort group of 107 adult AD patients who were hospitalized from 2009 to 2017. Examination of data included epidemiology, clinical and laboratory characteristics along with response to treatment.

The study results revealed that out of the total sample, 87 patients (i.e. 81%) were diagnosed with LOAD, and 20 patients (19%) were diagnosed with COAD. The median age for all patients was 66 years with 77 years for LOAD patients and 42 years for COAD patients. The incidence of atopy was less in the LOAD group than in the COAD group (33.8 vs. 68.8% for family history, 35.5 vs. 84.8% for personal history). A higher occurrence of head, neck, and flexural involvement was found in COAD patients. LOAD patients had lower immunoglobulin E levels and they responded well to phototherapy.
