Non-alcoholic fatty liver disease (NAFLD) is highly prevalent in the general population worldwide and is associated with high health burden. However, more studies are required to assess the prevalence and outcomes of NAFLD among elderly. The current study is aimed to evaluate the prevalence, risk factors and mortality of NAFLD in individuals older than 60 years.

The study used the data from the Third National Health and Nutrition Examination Survey with linked mortality files. NAFLD was typified according to United States Fatty Liver Index in the absence of other causes of liver disease. All-cause and cardiovascular (CV) mortality was determined by Cox proportional hazards models. All analyses were performed using SAS software.

The results declared that 3271 NHANES-III participants were recruited in the study. The prevalence rates of NAFLD were 40.3% and 39.2% among 60-74 and > 74 years old, respectively. The risks for 5-year and 10-year all-cause mortality were corelated with presence of NAFLD with adjusted hazard ratios (aHR) of 1.60 for 5-year and 1.22 for 10-year in patients aged 60-74. Additionally, CV mortality was reported to be higher in this age group, aHR was 2.12 for 5-year while it was 1.06 for 10-year. However, diagnosis of NAFLD was not related with all-cause or CVD mortality in individuals > 74 years old.

The study concluded that NAFLD is prevalent liver disease among elderly population. Furthermore, it is coorelated with augmented risk of mortality for 60-74 years old individuals. However, patients with NAFLD who are older than 74 years are not associated with increased risk of mortality.
