Predicting Noninvasive Mechanical Ventilation Failure in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease: Role of Neutrophil/Lymphocyte Ratio

Aim:

A study evaluated the effectiveness of neutrophil/lymphocyte ratio (NLR), in comparison with conventional inflammatory markers, to predict noninvasive mechanical ventilation (NIMV) failure in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD).

Methods: 

  • It was a retrospective observational study enrolling 212 AECOPD patients who needed NIMV during hospitalization.
  • The medical record was reviewed to assess the success of NIMV for each patient
  • NLR was compared with traditional markers (leukocyte, C-reactive protein [CRP] and procalcitonin [PCT]) for patients in NIMV failure and NIMV success groups.
  • The accuracy of these markers for predicting NIMV failure was assessed.

Results: 

Conclusion: 
  • The study deemed NLR to be an effective marker for predicting NIMV failure in AECOPD patients.
  • Additionally, the patients with NLR> 8.9 need more attention as they possess a higher risk of NIMV failure and require intubation.
