Liver Update: CLD in the extremely elderly of 80 years or more: clinical characteristics, prognosis and patient survival analysis

A study explains the clinical characteristics of patients with chronic liver disease aged >/=80 years, focusing on the factors affecting prognosis and carcinogenesis.

The study enrolled 135 patients aged >/=80 years and categorized them, according to the severity of fibrosis, into- 

  • Chronic liver disease without cirrhosis (non-LC) group
  • Cirrhosis (LC) group

The clinical characteristics and prognoses of each patient were evaluated and the following observations were made-

  • 1% of patients were in the LC group 
  • 5% of patients had hepatitis C virus. 
  • 4% of patients demonstrated the presence of various concomitant diseases 
  • 5% of patients scumbled to Liver-related deaths (died during observation), however, 53.8% among those suffered liver cancer at the time of death.
  • Cumulative survival rates in the non-LC group were 85.7% in the 5th year, and 69.4% in the 9th year.
  • Cumulative survival rates in the LC groups were 58.8% in the 5th year, and 19.4% in the 9th year.
  • Cumulative liver cancer appearance rates in the non-LC groups were 1.6% in the 1st year, 12.4% in the 5th year, and 12.4% in the 7th year.
  • Cumulative liver cancer appearance rates in the LC groups were 6.1% in the 1st year, 19.9% in the 5th year, and 32.0% in the 7th year.
  • The presence of liver cancer, platelet count, and the fibrotic stage was found to be independently associated with the survival period.
  • Alfa-fetoprotein and bilirubin were found to be independently associated with carcinogenesis.

Thus Cirrhosis was rendered to be the major risk factor affecting the prognosis. Furthermore, more attention should be given to concomitant diseases specific to advanced age.
