  • Ultrafiltration is selected for patients:

o Hospitalized with decompensated heart failure.

o Evidence of significant congestion.

o Failed or inadequate response to IV diuretics equal to at least 80 mg furosemide/day.

  • Treatment initiated within 24-48 hours of first dose of IV diuretics.
  • Peritoneal dialysis may be considered for long-term therapy.
  • Expectation should be realistic. Symptomatic relief may be achieved but mortality benefit may not accrue.
