Metformin Revisited

  1. Metformin is in extensive use since 1950s in India and other countries. It was introduced in the USA in 1995. The treatment algorithm of type 2 diabetes recommends its use after failure of lifestyle modification. More importantly, it continues to be used with a 2 or 3 drug oral antidiabetic drugs (OAD) regime, as well as insulin therapy.
  2. New uses of metformin include: Gestational diabetes (GDM), polycystic ovarian syndrome, improving gut flora and adjunct in cancer therapy.
  3. There are newer insights into the mechanism of action of metformin and genesis of its side effects and formulations, which could potentially mitigate the side effects.
