Management of T2DM with Heart Failure: A New Paradigm

One out of four DM patients has undiagnosed heart failure (HF). Both HF and diabetes are global epidemics with tremendous costs on the society. Although each begets the other, links between the two conditions are not fully elucidated. Several antidiabetic drugs have been implicated in worsening HF symptoms or prognosis. SGLT2i have favorable profile in HF patients. Several HF therapies, such as b-blockers and diuretics, might aggravate dysglycemia. ARNI might have favorable metabolic effects in patients with HF and diabetes. SGLT2i and ARNI can have synergistic effects via several shared mechanisms. A combination of SGLT2i and ARNI can definitely be a first-line management for a T2DM patient presenting with HF.
