ISG Guidelines on Diagnosis and Management of H. pylori

  • Non-invasive tests are appropriate for H. pylori eradication.
  • Serological tests have limited role in H. pylori management.
  • Before testing for H. pylori infection, PPI and antibiotics should be discontinued for at least 2 weeks and 4 weeks, respectively.
  • In clinical practice, when there is an indication UGIE, RUT is recommended as the first line diagnostic test. For RUT, one biopsy should be taken from the corpus and one from the antrum.
  • If there is a failure of second line therapy and endoscopy is considered, culture and standard anti-microbial testing is recommended.
  • Detection of H. pylori genetic factor and the study of host genetic polymorphism is not helpful in H. pylori infection.
