AKIs in ICUs in India

The findings from various studies conducted on acute kidney injury (AKI) in ICUs across India are:

  • AKI: Incidence, prognostic factors and outcome of patients in an ICU in a tertiary center: A prospective observational study - Incidence of AKI was 16.1% in critically-ill patients. In patients with AKI, 39.1% required hemodialysis and 28-day mortality was 49.5%. AKI patients with septic shock with vasopressors have a higher requirement of renal replacement therapy (RRT), especially as age increases. Underlying chronic illness such as hypertension and chronic kidney disease (CKD), need for mechanical ventilation (MV), vasopressor support and RRT are associated with increased 28-day mortality in AKI.
  • Epidemiology, outcomes and validation of RIFLE and AKIN criteria in AKI in critically ill patients: Indian perspective - Using the definitions of RIFLE and AKIN criteria, AKI is common in patients admitted to the Indian ICU and significantly increases 28-day mortality. APACHE-III score and the severity of AKI are strongly associated with increased 28-day mortality. Both RIFLE and AKIN criteria seem to reliably predict outcomes.
  • Impact of dialysis practice patterns on outcomes in AKI in ICU - Adoption of prolonged intermittent RRT (PIRRT) resulted in 37% reduction of utilization of continuous RRT in the patients; The mortality was better during the period of adoption of PIRRT, but this could not be attributed to PIRRT use; Early initiation of RRT for indications of anuria and metabolic acidosis may have resulted in better survival during later period.
  • Comparison of early continuous hemodiafiltration vs. delayed continuous hemodiafiltration in patients of AKI with septic shock - Among critically-ill patients with sepsis with AKI, early RRT compared with delayed initiation of RRT reduced mortality over the first 28 days.
