  1. There are no major studies to reassess the readmission rates, a major issue in current HF practice.
  2. It may be used in selected individuals only as add-on therapy albeit with well-known caveats like its narrow therapeutic window.
  3. A typical example may be a patient of systolic HF in AF with faster heart rate who is not responding adequately despite routine guideline directed therapies including RAAS modulators and b blockers.
