  • Electron microscopy (EM) is useful in 35-50% of renal disorders.
  • It would be ideal to reserve tissue for EM in every case and to process it when needed.
  • Disorders where EM is invaluable: Hereditary nephritis; Fibrillary deposition disorders; To confirm MCD; Dense deposit disease; Suspicion of chronic active ABMR; Recurrent GN in graft.
  • Disorders where EM would be helpful: Early MN; Primary vs. secondary MN; MPGN pattern disorders; Immune complex GN but no glomeruli in IF tissue; Proteinuria in a graft; Graft dysfunction after 6 months.
