Obstetric Anal Sphincter Injuries at Vaginal Delivery: Recently Published National Guidelines.

A new review article published in Obstetrical and Gynecological Survey compared recommendations for Obstetric anal sphincter injuries (OASISs) at vaginal delivery, from recently published national guidelines. The present review included the following guidelines: Royal College of Obstetricians and Gynaecologists – The Management of Third- and Fourth-Degree Perineal Tears; American College of Obstetricians and Gynecologists – Prevention and Management of Obstetric Lacerations at Vaginal Delivery; and Society of Obstetricians and Gynaecologists of Canada – Obstetrical Anal Sphincter Injuries (OASIS): Prevention, Recognition, and Repair. The findings were summarized as follows: for the prevention of OASIS, routine use of episiotomy is not recommended; warm perineal compresses and perineal massage during the second stage of labor have a protective role. In the management of OASIS, special care is necessary of the torn anorectal mucosa and internal and external anal sphincters, during the repair process. Additionally, postoperative use of broad-spectrum antibiotics, oral laxatives, and analgesia is recommended. It was stated that the findings may support efforts towards reduction of morbidity associated with OASIS.
