The goal of a new study published in the American Journal of Kidney Diseases evaluated whether gestational diabetes mellitus GDM is associated with incident chronic kidney disease CKD controlling for pre pregnancy risk factors for both conditions. In this prospective cohort study included 820 women within the age group of 18 30 years from the Coronary Artery Risk Development in Young Adults CARDIA Study in 1985 1986. The inclusion criteria were that the women should be nulliparous at enrollment have had delivered at least 1 pregnancy longer than 20 weeks gestation and had undergone kidney function measurements during 25 years of follow up. The findings revealed that during a mean follow up of 20.8 years 12.8 women developed CKD predominantly with increased urine albumin excretion. There was evidence of a GDM race interaction on CKD risk. Among the dark skinned women the adjusted hazard ratio HR for CKD was 1.96 in GDM compared with those without GDM whereas among lighter skinned women the HR was 0.65. Therefore it was inferred that GDM is associated with the subsequent development of albuminuria in dark skinned women in CARDIA.
