Clinical characteristics, symptoms and burden of psoriasis and atopic dermatitis in adults.

The purpose of a new study published in The British Journal of Dermatology was to describe and compare disease characteristics, life-style factors and disease burden in adults with psoriasis and atopic dermatitis (AD). Here, the Danish Skin Cohort was searched and registry data was linked to clinical and patient reported outcomes for 3,834 adults with dermatologist-verified psoriasis and an equal number with AD, as well as 2,946 general population adults. Participants were predominantly women and middle-aged. It was found that psoriasis patients usually reported disease onset during adulthood. However, there existed a distinct early incidence-peakin those with a positive family history or severe disease. Contrastingly, AD predominantly began in childhood with only a very discrete incidence peak in adulthood. Symptoms like scalp, extremity, chest and abdomen involvement were common to both diseases. Scalp/hairline, elbows, nails, intergluteal cleft, umbilicus, knees and legs were most frequently affected in psoriasis patients. While in AD, periocular, neck, ante-cubital fossae, back of the hands, interdigital areas and popliteal fossae were commonly affected. On the other hand, psoriasis (but not AD) patients were generally more overweight and obese, physically inactive and had a positive smoking history compared to the general population. Both diseases exhibited more frequent flares with increasing disease severity, with time; patients generally displayed uncontrolled disease despite being on systemic therapies. Furthermore, itch, skin pain and troubled sleep were much more severe in AD patients, whereas joint pain was more common in psoriasis patients. Thus, the results identified important similarities and differences in the clinical characteristics of adults with psoriasis and AD, which could assist clinicians in prioritizing and improving patient management.
