Nail involvement features in the pathogenesis of palmoplantar pustulosis

Pathogenesis of palmoplantar pustulosis (PPP) shares some nail features with plaque psoriasis; however, differences were observed in the commonly observed features and the distribution of involved nails.

Psoriatic nail changes are the clinical features commonly occurring in both diseases; palmoplantar pustulosis and plaque psoriasis (PsO). An understanding of the disease pathogenesis will lead to a correlation between both diseases. In a recent retrospective review of the medical records and photographs of patients diagnosed with PPP to understand the nail involvement features in PPP patients, the results showed that nail involvement was seen in 66.3% of the patients, and its severity was correlated with the severity of cutaneous lesions. Onycholysis and crumbling were more frequently seen in PPP than with PsO. Risk factors for nail involvement in PPP include female sex, younger age at diagnosis, and higher severity.
