Liver Update: Guidance for COVID-19 vaccination in patients with chronic liver disease

  • Patients with CLD who are getting antiviral therapy for HBV or HCV or medical therapy for primary biliary cholangitis or autoimmune hepatitis should not discontinue their medications while receiving the COVID-19 vaccines.
  • Patients with hepatocellular carcinoma undergoing locoregional or systemic therapy should also be vaccinated without disruption of their treatment. Nevertheless, patients with recent infections or fever should not receive the COVID-19 vaccine, until they are medically stable. 
  • mRNA and adenoviral vector COVID-19 vaccines are anticipated to have an effective and safety profile in immunosuppressed patients and should be administered based on their standard dose and schedule. 
  • LT candidates with CLD should be administered with a COVID-19 vaccine before transplantation to provide a suitable immune response. 
  • CLD patients receiving a COVID-19 vaccine may have local and systemic reactions including fever, myalgias, headache in the first 48 hours after vaccination. However, be cautious of respiratory symptoms or systemic symptoms, which are suggestive of COVID-19 and need further investigation. 
  • All patients with CLD, including vaccine recipients, should continue to lessen their risk of SARS-CoV2 exposure by following guidelines of masking, social distancing, hand washing, etc.
