There should be early use of invasive/functional hemodynamic monitoring to guide timely and appropriate resuscitation with fluids and inotropes. Timely institution of antibiotics is needed. Nutritional support is important. Removal  and avoidance of all nonessential and potentially nephrotoxic exposures. Plan for early initiation of renal replacement therapy - % fluid overload >10%.
