Tinea are superficial fungal infections usually triggered by dermatophytes. The present unusual case describes the involvement of N. gypsea in chronic tinea corporis. Tinea corporis is a superficial fungal infection of the skin caused by dermatophytes. Tinea corporis can be found globally. Luliconazole is a unique imidazole derivative, which has well-known in vitro efficiency against dermatophytes.  A one-week treatment with luliconazole 1% cream shows favorable clinical and mycological results in clinical trials for tinea corporis.

A 22-year-old male patient, came with a history of itchy skin lesions over the body, forearms and legs for 7 months. Lesions originally started in summer as a small papule and in due course developed to form large circinate plaques. Systemic and general investigation of the patient was unexceptional. On dermatological examination, distinct circinate, scaly plaques with erythematous papules of variable sizes were seen over the flexor aspect of the right forearm and lower left leg.

On additional examination, similar lesions were seen in the lower abdomen, groin and the gluteal region. The patient was put on topical 1 % luliconazole applied once daily for 3 weeks and oral terbinafine 250 mg once daily at bedtime for 3 weeks. The itching and provocative lesions resolved without any residual scars and relapse after 1-year post-treatment.

Topical luliconazole application along with systemic terbinafine during the early days abridged the fungal load and contributed to the alleviation of the severity of lesions.

Luliconazole is a novel broad-spectrum antifungal. Its antifungal capability has also been shown to exceed its commercial counterparts in vitro. Phase III vehicle-controlled studies revealed that luliconazole 1% was expressively more effective in quickly resolving the signs and symptoms of tinea corporis.
