A recent article published in the Journal of the Pediatric Infectious Diseases Society reported a case of an ex-full term 5-week-old female infant with fever 100.4oF. The source of the fever could not be determined; the baby then tested positive for severe acute respiratory syndrome coronavirus-2.

The child had a history of hydronephrosis and duplex kidney. She had been breastfeeding well and was otherwise seeming well. Her father was reported of having five days of fever, myalgias, mild cough, coryza and pharyngitis without shortness of breath, and with loss of taste and smell.

The infant had mild symptoms and was briefly hospitalized; he had somnolence but did not develop respiratory distress.

It was concluded that COVID-19 should be considered in the differential diagnosis for young infants presenting with fever without a source. Proper protective equipment must be provided to medical personnel. In addition, parents of SARS-CoV-2-positive children should be counseled on methods to avoid further community spread of the virus.
