Intravenous immunoglobulin therapy in COVID-19 related acute respiratory distress syndrome

Aim: A study investigated the efficacy and safety of IVIG in COVID-19 pneumonia patients needing invasive mechanical ventilation for moderate-to-severe ARDS.

Study design and methods:  

  • A retrospective study analyzed the electronic health records of COVID-19 patients admitted to intensive care units (ICUs).
  • Patients receiving invasive mechanical ventilation for moderate-to-severe ARDS were categorized into two groups- 

o Those receiving IVIG therapy 

o Those not receiving IVIG therapy. 

  • All-cause ICU mortality was the primary outcome while ventilator-free days and ICU-free days at day-28 and incidence of acute kidney injury (AKI) were secondary outcomes.
  • Propensity score matching and competing-risks survival analysis were performed.

Results

Conclusion

  • Mechanically ventilated patients with COVID-19 related moderate-to-severe ARDS in IVIG therapy were correlated with higher ICU mortality.
  • A randomized clinical trial is warranted to confirm this observation.
