Cardioprotection in Type 2 Diabetes Mellitus

  • The burden of cardiovascular disease (CVD) is very high in India. The risk is increased 2 to 4 times if an adult has diabetes mellitus.
  • Women with diabetes seem to lose their inherent protection against CVD.
  • The time to occurrence of the first MACE is shorter in South Asians compared to Europeans.
  • This is a continuum, starting from before prediabetes sets in and progresses further as type 2 diabetes develops.
  • The treatment for the prevention of CVD should be started early in patients with diabetes.
  • The drugs like SGLT2 inhibitors have multiple beneficial effects on CV and renal outcomes.
  • ADA 2021 recommends that among patients with type 2 diabetes who have established ASCVD or indicators of high-risk, established kidney disease or heart failure, SGLT2 inhibitors or GLP-1 receptor agonists with demonstrated CV benefits should be given.
  • The treatment should be initiated early to get the maximum benefit.
