Management of vitiligo during the COVID-19 pandemic: A survey and resulting consensus

Since there is a lack of consensus regarding ways to deal with patients of vitiligo in the ongoing pandemic scenario, a pair of researchers conducted a questionnaire-based survey, amongst expert dermatologists; to apprehend the influence of COVID-19 on the management of vitiligo.

A semi-structured English questionnaire was circulated among 50 pan-Indian expert dermatologists and their responses were analyzed, followed by the generation of a consensus statement.

The following results were obtained-

  • Teleconsultation was rendered adequate for the diagnosis and management of most cases of vitiligo by 42.22% of the respondents.
  • 64.44% believed in prescribing oral medications, even during the COVID-19 pandemic.
  • Among those who prescribed oral medications, 62% preferred systemic steroids; followed by 17%, 11%, 7%, and 3% who preferred cyclosporine, azathioprine, mycophenolate mofetil, and methotrexate, respectively.
  • In-office phototherapy was recommended by 64.44% of respondents.
  • Home-based phototherapy (PUVAsol or handheld NBUVB devices) was preferred by 80% of respondents.
  • 86.67% of respondents preferred stopping the oral immunosuppressive drugs, till the COVID-19 RTPCR positive patients are tested negative. 

The conclusion was made as-

  • An initial consultation may be seen physically, while follow-up may be scheduled for teleconsultation.
  • Topical therapy can be utilized without any reluctance.
  • Phototherapy may be reasonably advised with either PUVAsol or home-based phototherapy units (handheld NBUVB devices).
  • Concerning systemic immunosuppressives, oral minipulse therapy may be preferable because of the lower need for monitoring. Surgery for vitiligo should be performed only in cases of is severely affected psychological well-being.
