Tampering with the original State Medical Council registration certification amounts to professional misconduct. In an order DMC/DC/F14/2/Comp 363/2006 dated 19th December, 2006, the council took suo moto action against Dr RKG who tampered with his medical council registration certificate by changing his name from Dr RKG to Dr RM. A warning was issued with a stricture recorded in the State Medical Register of Delhi Medical Council.

One cannot change the name in the Council registration certificate. This is an official process and requires advertisement in the newspaper and Gazette notification, i.e. publishing the name in the official Gazette of the state after submitting a proper application to the government.
