The goal of a new study published in Dermatology was to assess the performance of high-frequency ultrasound (HFUS) in the evaluation of basal cell carcinomas (BCCs) to formulate recommendations for its uses.

This was a systematic review of the literature that entailed searches from Central, Medline, Embase, CINHAL and Web of Science. Overall, 30 articles and 1,203 biopsy-confirmed BCCs were included.

The results showed that HFUS provides accurate depth measurements, especially for BCCs >1 mm. The definition of lateral margins in vivo needed further accuracy, although ex vivo margin assessment was almost precise. Additionally, for HFUS can identify higher recurrence subtypes of BCC and can help in risk stratification. HFUS seems to be crucial in planning the management of BCC – it aids in case-selection for Mohs.

Furthermore, HFUS can improve safety when referred to before brachytherapy treatments and may also serve as an adjunct treatment choice pre-photodynamic therapy. Besides, HFUS can be used for follow-up assessments, especially in cases with nonsurgical management.

Moreover, HFUS can enhance clinical practice and decision-making by revealing important findings. It is recommended as a tool for evaluating the extent, depth and aggressiveness of the BCCs.
