Dapagliflozin well tolerated and Safe for Kidneys in Hospitalized COVID Patients

The DARE-19 trial disclosed that dapagliflozin (Farxiga), a sodium-glucose cotransporter-2 (SGLT2) inhibitor, was safe and had no adverse effect on renal outcomes in hospitalized COVID patients, regardless of their baseline estimated glomerular filtration rate (eGFR).

1250 patients (18% had eGFR <60 ml/min per 1.73 m2) with COVID-19 and cardiometabolic risk factors were included in the study and divided into groups receiving dapagliflozin or placebo. These patients had a supplemental oxygen saturation of 94 per cent and a history of hypertension, type 2 diabetes, atherosclerotic cardiovascular disease, and heart failure.

A similar occurrence for a composite kidney outcome of acute kidney injury (AKI), kidney replacement therapy, or death (HR 0.74, 95% CI 0.50-1.07) was observed in patients with cardiometabolic risk factors who were given dapagliflozin.

No difference was observed in the risk for the composite kidney outcome between baseline estimated glomerular filtration rate (eGFR), either <60 mL/min/1.73 m2: HR 0.91 (95% CI 0.49-1.69) or ≥60 mL/min/1.73 m2: HR 0.65 (95% CI 0.40-1.05).
