CNS Update: Pregabalin for neuropathic pain in primary care settings: recommendations for dosing and titration

Pregabalin remains the first-line treatment for managing neuropathic pain (NeP). Several patients are benefitted from pregabalin, however, they are usually treated with suboptimal doses, conceivably due to unawareness about prescribing the drug and/or side effects concerned with up-titration. 

A study described-

To guarantee the most significant therapeutic benefit to the patient, a low and slow dosing technique must be followed to confine common side effects and optimize tolerability seeing patients expectations. 

On titrating to higher doses, initiating asymmetric dosing, with the larger dose in the evening will be beneficial. 

Completely engaging patients in explaining the expected timeline for efficacy and side effects (including their resolution), will help individualize the optimal titration tempo. 

The low and slow approach also acknowledges that patients with NeP are heterogeneous regarding their optimal therapeutic dose of pregabalin. Thus health providers must closely monitor patients and up-titrate depending on pain relief and side effects to limit suboptimal dosing or early discontinuation.
