CNS Update: Efficacy and tolerability of Escitalopram in patients with CKD undergoing hemodialysis having depressive disorder

Depression is associated with poor quality of life, pain, and sleep parameters. Treatment of depression in patients with Chronic Kidney Disease will not only improve depression but also enhance the quality of life, and increase the length of life. Most psychotropic medications can be safely used in the Chronic Kidney Disease population. The majority of these drugs are fat soluble, cross the blood-brain barrier, non-dialyzable, metabolized in the liver and excreted through bile. 

Psychopharmacological trials with Escitalopram in patients with depressive disorder experiencing hemodialysis are mostly case reports and case series. End-Stage Renal Disease (ESRD) has minimal influence on the clearance of Escitalopram.

A recent study investigated the efficacy and tolerability of Escitalopram in patients with CKD undergoing hemodialysis with Depressive disorder.

Patients with CKD with a depressive disorder who were advised Escitalopram were evaluated for improvement in the symptoms and development of side effects, after the treatment course with Escitalopram in a dose ranging from 5-20 mg/day. They were evaluated on 4, 8, and 12 weeks during their follow-up by employing the Hindi Mental Status Examination (HMSE), Hamilton Depression Rating Scale (HAM-D), Global Assessment of Functioning Scale, Systematic Assessment for Treatment Emergent Events (SAFTEE SCALE), Epworth Sleepiness Scale, Pittsburgh Sleep Quality Index, which resulted in a significant reduction in the symptomatology of depression with very fewer tolerability issues.
