CNS Update: Coenzyme Q10 supplementation alleviates Pain in pregabalin-treated fibromyalgia patients

Evidence suggests a role of coenzyme Q10 (CoQ10) supplementation in reducing pain levels in chronic pain; however, its effect on Pain, anxiety, brain activity, mitochondrial oxidative stress, antioxidants, and inflammation in pregabalin-treated fibromyalgia (FM) patients remains unclear.

 

The present study hypothesized that CoQ10 supplementation reduces Pain better than pregabalin alone by reducing brain activity, mitochondrial oxidative stress, and inflammation and increasing antioxidant levels in pregabalin-treated FM patients. 

 

The study enrolled Eleven FM patients with two weeks of wash-out. It then randomized them into two treatment groups; pregabalin with CoQ10 or pregabalin with placebo for 40 d. It then switched patients in the CoQ10 group to placebo and patients in the placebo group to CoQ10 for another 40 d. 

 

The study examined Pain pressure threshold (PPT), FM questionnaire, anxiety, and pain score. It isolated Peripheral blood mononuclear cells (PBMCs) to investigate mitochondrial oxidative stress and inflammation at days 0, 40, and 80. It also determined the level of antioxidants and brain positron emission tomography (PET) scan at these time points. 

 

The study found that pregabalin alone reduced Pain and anxiety via decreasing brain activity compared with their baseline. Yet, it did not affect mitochondrial oxidative stress and inflammation. CoQ10 supplementation showed to effectively reduce greater Pain, anxiety, and brain activity, mitochondrial oxidative stress, and inflammation; and increase reduced glutathione levels and superoxide dismutase (SOD) levels in FM patients. 

These findings indicate that CoQ10 supplementation in pregabalin-treated FM patients further relieves pain sensation, possibly via improving mitochondrial function, reducing inflammation, and decreasing brain activity.
