A Neonate with Amniotic Band Syndrome

A report describes a case of a 23-year-old second gravida mother with 33 weeks of gestation who presented with active labor. She experienced spontaneous abortion two years back and was diagnosed case of situs inversus totalis. She described a history of hypothyroidism and was on levothyroxine for the past two years. The patient had a non-consanguineous marriage and no other significant ante-natal history. She received serial ultrasonograms ante-Nataly. 

 

Her sonography revealed fusion of amnion only at 20 weeks of gestation and a constriction band in the right leg along with resultant edema in the right foot at 28 weeks of gestation. She underwent emergency cesarean section (Indication: Breech presentation/Premature Rupture of membranes/Oligohydramnios) and delivered a male baby weighing 1500 grams. During the delivery, the right leg of the baby, just above the level of the ankle, got separated from the right foot, which was attached with a very narrow pedicle. The separated right leg and foot showed edema and focal areas of necrosis. There weren′t any other congenital anomalies, and the infant′s Postnatal screening echocardiogram, cranial ultrasonogram, and abdominal ultrasonogram were normal. The baby received regular neonatal care and a discharge on the 12th day. 

 

He was a case of Amniotic band syndrome (ABS) or Streeter′s dysplasia. Children with auto amputation are managed with reconstructive surgery or prosthesis.
