Researchers from India have Created a New Mathematical Model to Estimate TB Burden Cases

New Delhi: According to official sources in the Health Ministry, India is the first country to create its own mathematical model to calculate the burden of tuberculosis cases. It has never been simple to estimate the burden of tuberculosis (TB), particularly in terms of incidence and mortality. This is especially true when reporting is aggregated or when the case-based monitoring systems coverage is still not at saturation levels.

However, since Nikshay (a case-based web-based surveillance system) was introduced and rapidly adopted in India in 2012, the quality of information and case notification has steadily improved. Moreover, several efforts were made over the past nine to ten years to broaden program coverage, track down unaccounted-for instances, and produce additional proof. The official source revealed that active case discovery has reduced the communitys per capita transmission rate by assisting in community screening for vulnerable individuals and providing an opportunity for early diagnosis.

The sources also noted that the country also implemented a daily regimen for the first-line treatment of tuberculosis in 2017, coordinating the regimens across the public and private sectors to improve access to TB care for all people. Tracking patients through Nikshay has also aided in improving treatment outcomes for all types of patients in the public and private sectors, including those who might have otherwise been lost to follow-up. The 2020–21 National TB Prevalence Survey provided estimates at the state level that allowed for the identification and measurement of the diversity in TB burden across the nation.

The new model was created using data from the disease′s natural history, each persons infection and state, their use of healthcare services, any missed or accurate diagnoses, the extent of their treatment, and their response to treatment, including whether they recovered or passed away. The model was then calibrated using the observed or reported figures based on the prevalence rate, notification rate, mortality rate, proportion of patients being placed on treatment, reduction in notification over time in the public and private sectors, and proportion of latent TB infection (LTBI) in the community.
