  1. Mother/primary caregiver with active cavities
  2. Parent/caregiver with low socioeconomic status
  3. Prolonged breastfeeding or bottle-feeding (>12 months)
  4. Frequent consumption of sugary beverages and snacks
  5. Use of a bottle at bed time, especially with sweetened beverages
  6. Use of liquid medication for longer than three weeks
  7. Exposure to passive tobacco smoke
  8. Children with special health care needs
  9. Insufficient fluoride exposure
  10. Visible plaque on upper front teeth
  11. Enamel pits or defects
