NASH: Standard of Care in 2018

  • Diet and exercise are recommended for patients with NAFLD. Evidence suggests that this measure can aid in reduction of liver fat and/or liver aminotransferase concentration.
  • Histopathological evidence suggests that lifestyle modification can help in reducing inflammation and fibrosis in the hepatic cells of these patients. Aerobic exercise training reduces hepatic and visceral lipids in obese individuals without weight loss.
  • Foregut bariatric surgery can be considered for otherwise eligible obese individuals with NAFLD or NASH. But, it is premature to consider surgery as an established option to specifically treat NASH.
  • Management of NASH, cryptogenic cirrhosis, and HCC are largely the same and includes surveillance for HCC.
  • Patients should be differentiated, whether having NASH or NAFLD; exercise is recommended for all patients while weight reduction is recommended for overweight/obese individuals.
