Leg Ulcers: Myriad Presentations and Practical Approach

  • Leg ulcers are an orphan disease area which needs multifocal approach by a team of dermatologists, surgeons, physicians, podiatrists, nutritionists, nurse care, etc.
  • The most common ulcers of leg are venous leg ulcers followed by arterial, diabetic, trophic and lymphoedematous ulcers.
  • The bare minimum work-up of a leg ulcer includes an ankle brachial pressure index (ABPI) assessment, venous and arterial Doppler, hematology and biochemistry investigations, albumin, blood sugar, culture AST, biopsy/direct immunofluorescence (DIF) and urinalysis.
  • Advanced work-up must cover vasculitis, livedoid vasculopathy, pyoderma gangrenosum, drug history, thrombophilia screen and may be genetic screens.
  • In any case of nonhealing leg ulcers, the inclusion of a thrombophilia screen is desirable, if not mandatory.

Practice sutra: “The eyes see what the mind knows” must be the adage for practicing dermatologists to upgrade their knowledge on the common and uncommon pathologies of this persistent malady.
